Make sure that the only active template in Word is the officefw2007.dot addin from MatterSphere, remove all others.
Updated for Office365 - 04
Close Word & then check the folder c:\Program Files (x86)\Microsoft Office\root\Office16\Startup the only template that should be present is officefw2007.dot
Launch Word & test
